Captive nut and plate ID

Please can someone give me the correct name for the items pictured,I’m struggling with the diagram that has been put up for Alan Re Bolt positions on his 13/60 as I’m in a similar place. I would like to know the correct name of these items and if possible the best place to obtain some replacements as looking at the diagram I think I need 8 yet only 2 fell off the car when dismantling!
